- No discharge pipe at all, leaving the valve to vent scalding water at head height
- The pipe terminating too high above the floor, so the discharge sprays rather than runs
- A reduced diameter, narrower than the valve outlet
- Threading, a cap or a valve on the end, which defeats the entire purpose and is the most serious version
- The pipe running uphill, so it cannot drain and holds water
- The wrong material for the temperature involved

Expansion tanks, and why they turn up so often now
This is the item most homeowners have never heard of and the one that appears on an increasing number of reports.
Water expands when heated. In an open system that expansion pushes harmlessly back into the supply main. Where a pressure reducing valve, a backflow preventer or a check valve has been fitted, the system becomes closed, and the expanding water has nowhere to go. Pressure rises every heating cycle.
An expansion tank is a small vessel, usually mounted on the cold supply near the heater, containing a diaphragm with an air cushion behind it. The air compresses, the pressure stays controlled, and the relief valve is not asked to do a job it was not designed for.

Combustion air and garage installations
Gas units need adequate air for combustion, and a heater sealed into a small cupboard without provision for it is a standard finding. In garages, requirements about ignition source height above the floor exist because petrol vapour is heavier than air, and inspectors do look at it.
Dielectric unions and mixed metals
Where copper meets the steel tank fitting, dissimilar metals corrode at the joint. Green or white crusting at the top connections is the visible sign. Minor, common, cheap, and one more line on the list.
Electrical items on electric units
Missing or unsecured cover plates over the element access, unsupported flexible conduit, and no accessible disconnect. All small, all reportable.
Working out whether the heater itself is near the end
Typical service life for a conventional storage heater is commonly cited as around eight to twelve years, with water conditions and whether it was ever flushed making a large difference either way.
The manufacture date is encoded in the serial number on the rating plate, and the format differs by manufacturer. Many use the first four digits as year and week, but not all do, and the manufacturer's own decoder is the reliable route. Do not guess from the install date; heaters sit in warehouses.
Signs it is near the end: rusty water from the hot side only, a metallic taste, rumbling or popping during heating which indicates sediment, visibly reduced hot water volume, moisture or rust at the base, or any weeping from the tank body itself. A leak from the tank body is not repairable and means replacement.
